Program Sponsor: KerixHealth

1) Program overview The KerixHealth Referral Program (“Program”) lets eligible KerixHealth members (“Referrers”) refer new individuals (“Referred Members”) for KerixHealth membership at participating locations. If a referral qualifies, the Referrer receives a one-month credit back to the Referrer’s membership account (the “Referral Credit”), subject to these Terms.

2) Key definitions Referrer: An eligible current KerixHealth member who submits a referral or shares a referral link/code. Referred Member: An individual who is not currently enrolled as a KerixHealth member and who enrolls as a new member through a qualifying referral. Qualifying Referral: A referral that meets the eligibility requirements in these Terms and where the Referred Member completes their first full month of membership. Referral Credit: A credit equal to one month of the Referrer’s membership fee, applied to the Referrer’s membership account (not cash, not a prepaid month).

3) Eligibility To participate as a Referrer, you must: Be a current KerixHealth member in good standing at the time the Referral Credit is issued, and Participate through a participating location (see Section 9). To qualify as a Referred Member, the individual must: Be a new member (not currently enrolled with KerixHealth at the time of enrollment), and Enroll as a paid member through a participating location, and Complete their first full month of membership (see Section

5). KerixHealth may refuse to issue Referral Credits where we reasonably believe the referral violates these Terms or is associated with fraud, abuse, or suspicious activity.

Addendum to Section 3) Eligibility — Employer-Sponsored Members Employer-Sponsored Members (Referrers).
Members enrolled in KerixHealth through an employer-sponsored program are eligible to participate as Referrers, including referring friends and family, provided they are current members in good standing and the referral is made through a participating location. Benefit for Employer-Sponsored Referrers.
Because employer-sponsored memberships are paid by the member’s employer, eligible employer-sponsored Referrers will receive a $100 gift card in place of the one-month credit back to a membership account. The $100 gift card will be issued only after the Referred Member becomes a Qualifying Referral (i.e., enrolls as a paid member through a participating location and completes their first full month of membership), consistent with Sections 5–7.

All Other Terms Apply.
Except for the Referrer benefit described above, all eligibility requirements, qualifying conditions, and Program Terms & Conditions apply to employer-sponsored Referrers and their referrals, including fraud/abuse protections and participating location requirements.  

4) How to make a referral Referrals must be submitted and tracked through KerixHealth’s designated referral process (for example, referral link/code and/or referral submission form). A referral must include enough information for KerixHealth to reasonably identify the Referrer and match them to the Referred Member.

5) When a referral qualifies A referral becomes a Qualifying Referral only after the Referred Member: Enrolls as a new member at a participating location, and Completes three full month of membership. If the Referred Member cancels, fails to pay, or otherwise does not complete three full month of membership, the referral will not qualify and no Referral Credit will be issued.

6) What the Referrer receives (important) For each Qualifying Referral, the Referrer receives a one-month credit back to the Referrer’s membership account. The Referral Credit is not provided upfront. The Referral Credit is applied after the Referred Member completes their first full month of membership. The Referral Credit is a credit to the Referrer’s membership account and is not redeemable for cash, not transferable, and not a gift card.

7) How and when the credit is applied Referral Credits are typically applied to the Referrer’s account on the next available billing cycle after the Qualifying Referral is confirmed. The credit will be applied as an account credit against future membership charges. If the Referrer’s membership is paused, canceled, or otherwise not in good standing at the time the credit would be applied, KerixHealth may delay or decline issuance of the Referral Credit.

8) Family memberships (important) If a Referrer’s membership includes multiple family members, only the Referrer (the member who submitted the referral or whose referral link/code was used) will receive the one-month credit back to their membership account. No additional credits are issued to other family members on the same membership.

9) Participating locations The Program is available only at participating KerixHealth locations and may not be offered in all areas or through all partner arrangements. Program availability can change at any time. If a location does not participate, referrals associated with enrollment through that location may be ineligible.

10) Limits, abuse, and prohibited activity The Program is intended for personal, lawful referrals. Prohibited conduct includes (but is not limited to): Creating fake accounts or referring the same person multiple times in a deceptive way Posting referral offers in a manner that violates applicable law or platform rules (e.g., spam) Misrepresenting KerixHealth services, pricing, or terms to induce enrollment Any fraud, abuse, or suspicious activity KerixHealth may withhold or revoke Referral Credits and/or suspend participation if we believe these Terms have been violated.

11) Taxes Referral Credits may be taxable depending on your circumstances. You are responsible for any taxes related to Program participation.

12) Privacy Information provided through the referral process will be used to administer the Program and match referrals. KerixHealth will handle personal information in accordance with our privacy practices and applicable law.

13) Changes or termination KerixHealth may modify, suspend, or end the Program at any time, including changing eligibility rules or credit amounts, to the extent permitted by law. Changes apply prospectively from the effective date of the updated Terms.

14) Disclaimers The Program is void where prohibited or restricted by law. KerixHealth is not responsible for technical issues that prevent referral tracking (e.g., link blocking, browser settings, ad blockers). KerixHealth’s decisions regarding Program eligibility and referral validation are final to the extent permitted by law.
